Dear Partner,

Credible's Technical Team plans to push enhancements and resolutions for the following items this Thursday night, November 12, between 10:15 PM - 12:15 AM ET. This list is a continuation of the items that were slated for last week and were pushed due to additional quality assurance testing needed.

New Signature Capture Method in Credible:

Last week, Credible sent out a communication regarding a new, streamlined method for capturing signatures in our software. This functionality will be released on November 12, and allows utilization of Topaz signature pads. This replaces all previous methods for capturing signatures in Credible.

Scheduled Enhancements & Task Resolutions for November 12:

Next Gen mobile visits missing ICD-10 coding: Credible corrected an issue where visits uploaded from Next Gen Mobile were not being coded to ICD-10 even though the client had a diagnosis with an ICD-10 code.
Medication eligibility fails: An issue was corrected where Medication Eligibility would return a Subscriber/Insured Not Found error even though the client does have coverage with the insurance.
Logged out when clicking the Reports tab: An issue was corrected where users would sometimes be logged out when clicking the Reports tab.

Wrong medication getting refilled when refilling a prescription order: When two users are simultaneously updating the same client's physician's orders, sometimes the wrong order would be updated. Now, the correct physician's order is updated even if another user is updating an order for the same client at the same time.
Syntax error with problem list form mapping: When adding "(307.23/F95.2) Tourette's disorder" in a form, the error 'incorrect syntax near 's'' would appear when saving the visit. Problem list form mapping now correctly handles apostrophes in the diagnosis description.

Timeout error when generating Bed Board visits: Credible resolved an issue where generating bed board billing visits would sometimes time out.
Tx Plus fails to expand inside of a visit: An issue was corrected where Tx Plus Plan would not expand when viewed inside a form.

Don't send visit date in institutional claims: Added two new Billing Payer settings for institutional billing:
  • When 'Do Not Send Service Date in 837I' is checked, the 2400 Loop DTP*472 segment (Service Date) will not be sent in an 837 I file.
  • When 'Box 45 Blank' is checked, Box 45 (Serv. Date) will be left blank on the CMS 1450.
Credible plans to push these enhancements Thursday, November 12 between 10:15 PM - 12:15 AM ET. There may be intermittent service during this time.

Credible's Technical Team plans to push enhancements and resolutions for the following items tonight, Thursday, November 5, between 10:15 PM - 12:15 AM ET.

Enhancements and Task Resolutions Thursday Night

Update - Upcoming Enhancements and Task Resolutions

Dear Partner,

Credible's Technical Team plans to push enhancements and resolutions for items mentioned below Thursday night, June 25th between 10:15 PM - 11:15 PM ET. 

New Scanner Plugin for all Browsers: Beginning with version 45 (slated for later this year), Chrome is ending support for NPAPI plugins, like the scanner plugin. With other browsers likely to follow suit, a new scanner plugin will be available. To access the new plugin:
  1. Go to your employee page
  2. Click 'config' on the nav bar
  3. If the insurance card / attachment scanner box is checked, you will see a new link: 'Test device using plugin version 10.2'
  4. Clicking the link will take you to a test page where you will be prompted to install the plugin
  5. Follow the installation prompts
You will not need to uninstall the old plugin - both can exist in your browser at the same time. However, Credible will remove the old scanner plugin from production in July. Once support for the old plugin is removed, scanners will only work with the new plugin. All Partner staff using scanners should install the new plugin by July 9th.

Ability to Sign eLabs: Credible added the ability for users to electronically sign eLabs results using their password. Only users with the new eLabsSign security right will be able to sign lab results.

 
Continuation of ICD-10 Enhancements:
  • Axis 2 diagnoses that are NOT DSM-5 will no longer copy forward to the problem list.
  • Added logging for re-sequencing the list of diagnosis
  • Added a view - 'ICD10View' - to allow creation of exports using the master list of ICD-10 codes & descriptions
  • Added ability to import diagnosis information directly into the problem list format

New Payer Configuarion Setting: Added a new payer configuration setting - 'Do Not Send Provider NPI or Taxonomy in 2310B loop for specified POS'. To use the new setting, select the 'place of service' from the drop-down. When the visit location's place of service matches the payer setting, the rendering provider's NPI and taxonomy code will not be sent in that claim. Claims for other places of service will still send the NPI and taxonomy code.

Insurance Payment Timeouts: Credible resolved an issue where a timeout error was received when updating an insurance payment that had been applied to over 1,000 services. The account codes for unbatched ledger lines are recalculated whenever the payment is updated. This was occurring even if the update would not affect the revenue code (changing the deposit date, adding text to the note) - the accounts codes will now only recalculate if there is a change to the payer or to the payment category/location.

 
Med/Med & Med/Allergy Interaction: The Med / Med - Med/Allergy interaction checks are now performed when adding a medication with the 'Add Medication' button. As with the interaction checks performed when prescribing using CredibleRx, only medications & allergies that were selected using the dropdown can be reviewed for interactions. If a Med/Med or Med/Allergy interaction is found, a popup will display the interaction warning and provide the option to continue adding the Medication or to cancel.

Credible System Update to Yesterday's Access Issues

Dear Partner,


Yesterday, Credible fully understands was challenging for you, your Agency, care providers, and clients.  We fully understand the criticality of our software and the role it plays with your Agency.

Starting at approximately 10:45 a.m. yesterday, Credible experienced an issue that severely affected our environment.  Based on our real time monitoring system that showed an abnormal and severe spike and volume of traffic - all the hallmarks of a DDOS attack, we concluded it to be so.  However upon further investigation, it was a false positive and merely a symptom of another issue.   Please rest assured that we spared no expense, effort and retained experienced outside experts to find the root cause.  That said, the source of the event was one of our main storage arrays becoming unresponsive due to several factors - suboptimal de-duplication of data, storage misallocations, write cache errors, and now adjusted monitoring levels.   If only one of these factors occurred it would have been transparent, however because they transpired simultaneously - it adversely affected performance and availability.
We truly apologize for the service interruption and the time it took to recover - please understand that we needed to be thorough, take great care to safeguard the integrity of the data, and needed to determine performance would be consistent once the environment is operational. 

We are committed to providing the best user experience, and we aren't stopping here. Credible will be engaging specific vendors to further review the environment to identify any nascent exposures, as well as working together to collapse restart times and processes. This may require us to replace or upgrade certain key components to mitigate any vulnerabilities and ensure quality of services. To focus on hardening our production environment, we will be postponing our ICD - 10 release on May 21st to May 28th.  We will send out the proper notification so you may prepare for any disruptions.

Crediblebh.com came under a Denial of Service Attack

Dear Partner,

Credible IT has verified that this morning Crediblebh.com came under a Denial of Service Attack from an unknown source or actors.  While all data remained secure, Partners access was limited or blocked during the DDOS attack.

We anticipate having full service restored as soon as possible.  Internal and external teams are focused on three areas:
  1. Restoring full access and service,
  2. Hardening hardware and software capabilities to mitigate ever evolving DOS attacks, and
  3. Analyzing the data and logs we have on the attack beginning this morning. 
All data remains protected and secure during this event.

Transition to ICD-10: Key Announcements and Dates

Dear Partner, 

To assist your Agency's transition to ICD-10, Credible has summarized key announcements and dates below. 

Credible is pleased to confirm that we can provide DSM-5 descriptions with specifiers for $15 per user. Credible is subsidizing 50% of the user fee, thus, Agency cost is $7.50 per user. Now pricing is confirmed, Credible will be sending out the DSM-5 order forms to Partners that requested it. Please note: If necessary, Partners may decide at a later date to add DSM-5. 

Due to APA delays, DSM-5 will not be available to those Partners that have requested May 21st. Credible is working with the APA in an effort to have this available to Partners as soon as possible (June 2015) and will keep Partners updated on the status of DSM-5 availability. Please note: Partners can still begin their ICD-10 transition without the DSM-5 codes present. 

ICD-10 enhancements will be released Thursday, May 21st.

ICD-10 Enhancements Released May 21st Include:
  • Problem List Format (Master Level Partner Config - submit request to Credible to turn on in Live Domain when ready)
  • Ability to add ICD-10 codes to all Active Client Diagnoses (Partner can also choose to have Credible do the GEMs 1:1 mapping, if desired.)
  • ICD-10 Mapping Client Detail Report (Provides list of clients with an active Dx without an ICD-10 code)
  • ICD-10 Mapping List Report (Provides list of Dx being used that does not have a defaulted ICD-10 code)
Key Dates

May 20 - Partners that wish to have Credible update ICD-9 codes to ICD-10s (where a 1:1 match exists) in Partner's Live site need to return sign off form by this date. Please note: Partners may still request GEMs mapping at a later date, but it will not be completed in conjunction with the ICD-10 Enhancements being released.

May 21 - Credible will push all ICD-10 enhancements to Partners' Live sites. Please note: Credible recommends Agency complete testing in Credible's test domain or Agency's test domain prior to turning on and/or utilizing these enhancements.   

May 22 - Credible ICD-10 training webinar for Partners (register here).

October 1 - ICD-10/DSM-5 Transition mandated by this date.
